Help with court forms

Forms for use in the Supreme Court and High Court are set out in the Appendices to the Rules of the Superior Courts.
Forms for use in the Circuit Court are set out in the Schedules to the Circuit Court Rules.
Forms for use in the District Court are set out in the Schedules to the District Court Rules.
These forms may be copied by selecting the text and pasting to your blank document.

We have copied some of the most commonly used forms from the various Rules of Court and formatted them to make them easy to complete electronically. You can access these downloadable forms by clicking the links above.

Notes on the completion of forms

Microsoft Word (MS Word) forms;

1. To save the form to your hard disk, click on the file icon.
Some browsers allow you to view MS Word documents within the browser.
If your browser opens the form and you want to save it to your hard disk, right-click on the file icon and select 'Save Target As...'

2. Only those sections of the form to be filled in are accessible. Therefore the cursor will move from one fillable section to the next. If you click the cursor in a section and select the information text in it you can then type or paste your data over the selected text. There is no limit to the amount of data you may place in any section.

If the fillable sections of on the form are not shaded, you may click the Form Field Shading icon on the Forms toolbar to highlight those areas of the form.

Information / help text is provided at each fillable section and on the status bar below or by pressing F1 key. The word or in bold italics indicates you have options. You should delete the options you do not require.

Adobe Acrobat (PDF) forms;

For best usage you need to have Adobe Acrobat Reader 8 or higher.
1. To save the PDF form to your hard disk, right-click on the file icon and 'Save Target As...'
To open the form within the browser, click on the file icon.

2. You may complete the PDF form in two ways;
- By printing it and filling in manually, or
- By filling in 'on screen' and printing it.

To fill in 'on screen'
Click in the first grey frame and enter the required data.
Continue through the form using the TAB button on your keyboard.
After you have made your final entry, click outside the frame and print.

A tips note will open automatically when you open the form. To move this note click on name bar and drag.
To close this note click on the X. To re-open this note double-click on the small note icon.
This small icon will appear on the printed form but this is ok.

Completed forms should be submitted in the normal way to the relevant court office.

Questions and answers on downloadable court forms

Q. MS Word forms are locked and I can't change the main text of the form

A: The MS Word forms are deliberately locked so that users of these forms are able to alter only those parts of the forms that are required to be completed.

Q. MS Word forms don't print out to full A4 size

A: Initially forms are not full page length if printed out. However, once the fillable portions are completed the pages do fill out. MS word versions are intended to be used by people typingthem, not attempting to hand write after printing them out.

Q. The text size on the MS Word form is too small

A: If you have concerns about the type size open the document at 120% plus.

Q. How do I use the PDF form?

A: You may complete pdf forms in two ways; by printing the form and filling in manually or by filling in ‘on screen’ and printing it.  [Note: You cannot save data typed into the form].

Q. When I try to download/print the PDF form I encounter problems
